INTRODUCCIÓN: La actinomicosis es una enfermedad infecciosa poco frecuente, ocasionada por una bacteria Gram positiva. La especie más común es Actinomyces israelii. Dentro de sus formas de presentación, la torácica es la menos frecuente. PRESENTACIÓN DE CASOS: Reportamos dos pacientes con actinomicosis torácica de 8 y 13 años de diferentes zonas geográficas de Perú. El primer caso tuvo empiema necessitatis y el segundo, consolidación pulmonar y hemoptisis recurrente. Ambos tuvieron cierto grado de dificultad en su diagnóstico, pero con una buena respuesta al tratamiento antibiótico y quirúrgico. El diagnóstico fue mediante estudio histopatológico. Sin embargo, no se pudo identificar la especie de Actinomyces. CONCLUSIÓN: La actinomicosis torácica es poco frecuente en niños y se presenta como una lesión parenquimal con posible fistulización a la pared torácica. Este es uno de los pocos casos reportados en la literatura peruana, constituyendo una contribución al conocimiento de esta enfermedad y su manejo en pediatría.


INTRODUCTION: Actinomycosis is a rare infectious disease caused by Gram-positive bacteria. The most common species is Actinomyces israelii. Among its forms of presentation, the thoracic is the least frequent. CASE PRESENTATION: We report two patients with thoracic actinomycosis, 8 and 13 years old, from different geographical areas of Peru. The first case had empyema necessitans and the second, lung consolidation and recurrent hemoptysis. Both had a certain degree of difficulty in their diagnosis but responded favorably to antibiotics and surgical treatment. The diagnosis was based on the histopathological study. However, we were not able to ascertain the species of actinomyces. CONCLUSION: Thoracic actinomycosis is rare in children and presents as a parenchymal lesion with possible fistulization to the chest wall. This article is one of the few in the Peruvian literature, constituting a contribution to the knowledge of the disease and its management in pediatrics.

Resumen Introducción: La actinomicosis, es una infección crónica rara producida por bacterias del género Actinomyces sp. La afectación pélvica es una de sus formas más infrecuentes y en gran parte de los casos se relaciona al uso de un dispositivo intrauterino de larga data o a una cirugía previa. Como otras enfermedades raras, la infección es conocida como "la gran imitadora" por su variada forma de presentación y particular comportamiento pudiendo simular una neoplasia. El tratamiento es fundamentalmente médico y de buenos resultados. Caso Clínico: Damos a conocer el caso de una paciente que se presentó con un cuadro compatible con un tumor de recto, pero que resultó ser actinomicosis. El diagnóstico se realizó en base a la tinción de Gram, el cuadro clínico y el antecedente de un dispositivo intrauterino abandonado por más de 25 años. Fue corroborado posteriormente mediante anatomía patológica y tratada en forma exitosa con antibióticos por un periodo extendido. Conclusión: Si bien la actinomicosis es una patología infrecuente, debe ser considerada en el diagnóstico diferencial de los pacientes que se presentan con tumores de la pelvis. Un alto índice de sospecha y una actitud diagnóstica activa son fundamentales para un tratamiento oportuno, seguro y eficaz de esta enfermedad.


Introduction: Actinomycosis is a rare chronic infection caused by bacterias of the genus Actinomyces sp. Pelvic involvement is one of its most infrequent forms and in many cases it is related to the use of a longstanding intrauterine device or a previous surgery. Like other rare diseases, the infection is known as "the great imitator" because of its varied form of presentation and its particular behavior, which can simulate a neoplasm. The treatment is fundamentally medical with good results. Case Report: We present the case of a patient who presented with a rectal tumor but that turned out to be Actinomycosis. The diagnosis was made based on the Gram stain, the clinical presentation and the history of an intrauterine device left for more than 25 years. It was subsequently corroborated by pathological anatomy and successfully treated with antibiotics for an extended period. Conclusion: Although actinomycosis is an infrequent pathology, it should be considered in the differential diagnosis of patients who present with tumors of the pelvis. An active diagnostic attitude and a high index of suspicion are fundamental for the timely, safe and effective treatment of this disease.

Resumen Se presenta un caso clínico de un actinomicetoma plantar en un paciente sin factores de riesgo, cuyo diagnóstico fue realizado mediante una biopsia de tejido plantar por sospecha de una neoplasia. Dado que el paciente no respondió satisfactoriamente a la terapia de primera línea, debió completar 24 semanas de tratamiento con doxiciclina, a lo cual evolucionó favorablemente. Finalmente, se desarrolla una breve discusión sobre los micetomas plantares.


A case of plantar actinomycetoma without risk factors is presented, which was diagnosed by hystopatological analysis of a foot biopsy because of the suspicion of neoplasia. Since the patient did not fully respond to the first-line therapy antibiotics, a 24-weeks doxycycline regime was started, achieving a satisfactory response. Finally, a brief discussion on plantar mycetomas is presented.

Abstract Asymptomatic female genital tract colonization with Actinomyces spp is not uncommon, particularly among intrauterine device users. Pelvic actinomycosis is an extremely rare disease. The clinical picture can resemble an advanced ovarian malignancy. We report a case of pelvic actinomycosis mimicking ovarian malignancy diagnosed postoperatively. Preoperative diagnosis is possible if there is a high index of suspicion, obviating extensive surgery and preserving fertility, since long term antibiotic treatment can be completely effective. Pelvic actinomycosis should be included in the differential diagnosis of women presenting a pelvic mass, especially if there is intrauterine device use history.


Resumo A colonização assintomática do aparelho genital feminino por Actinomyces spp não é infrequente, sobretudo em utilizadoras de dispositivo intra-uterino. A actinomicose pélvica é uma doença extremamente rara. O quadro clínico pode assemelhar-se ao de uma neoplasia maligna do ovário avançada. Relatamos um caso de actinomicose pélvica, simulando uma neoplasia maligna do ovário, com diagnóstico pós-operatório. O diagnóstico pré-operatório é possível se houver um elevado grau de suspeição, permitindo evitar cirurgias extensas e preservar a fertilidade, uma vez que o tratamento antibiótico prolongado pode ser totalmente eficaz. A actinomicose pélvica deve ser incluída no diagnóstico diferencial da mulher que apresente uma massa pélvica, sobretudo se houver história de uso de dispositivo intra-uterino.

RESUMEN Objetivos. Aislar, seleccionar e identificar actinomicetos asociados a hormigas cortadoras de hojas Atta cephalotes (Linnaeus, 1758), que presenten mayor actividad anti-Candida. Materiales y métodos. Estudio transversal realizado en hormigas recolectadas de una localidad de Huánuco, Perú, a partir de las cuales se aislaron cepas de actinomicetos que fueron evaluadas mediante pruebas in vitro para determinar su capacidad antagonista frente a especies de Candida. Los actinomicetos de mayor antagonismo fueron seleccionados y cultivados en agitación, luego se obtuvieron los metabolitos extracelulares con solventes orgánicos y finalmente se evaluaron los extractos crudos para determinar cuantitativamente la concentración mínima inhibitoria (CMI). Resultados. Se logró aislar 30 actinomicetos, de los cuales el 47 % presentaron actividad antagonista a Candida albicans (C. albicans) ATCC 7516, el 43 % a Candida parapsilosis ATCC 7307, el 37% a Candida tropicalis ATCC 7206 y C. albicans ATCC 10231 y el 30% a C. albicans ATCC 98028. Extractos orgánicos de las cepas HAA-16 y HAA-17 presentaron marcada actividad anti-Candida; siendo el extracto de acetato de etilo de la cepa HAA17 el de mejor rendimiento por tener mayor espectro de actividad y presentar una CMI de 3,25 mg/ml frente a C. albicans ATCC 7516 y Candida parapsilosis ATCC 7307. Los actinomicetos seleccionados se identificaron mediante técnicas moleculares como miembros del género Streptomyces. Conclusiones. Los actinomicetos asociados a Atta cephalotes son excelentes productores de compuestos bioactivos, capaces de inhibir el crecimiento de levaduras patógenas del género Candida y con potencial aplicación en el desarrollo de nuevos productos naturales de interés biomédico.


ABSTRACT Objectives. To isolate, select and identify actinomyces associated to leaf-cutting ants Atta cephalotes (Linnaeus, 1758), that present a greater anti-Candida activity. Materials and Methods. Cross-sectional study made with ants collected at a location in Huánuco, Peru, from which strands of actinomyces were isolated and later evaluated by in vitro testing in order to determine its antagonistic capacity against species of Candida. The actinomyces with greater antagonism were selected and cultured by agitation, then the reliable extracellular metabolites were obtained with organic solvents, and finally the crude extracts were evaluated to determine quantitatively the minimum inhibiting concentration (MIC). Results. Thirty (30) actinomyces were isolated, of which 47% exhibited antagonistic activity against Candida albicans (C. albicans) ATCC 7516, 43% to Candida parapsilosis ATCC 7307, 37% to Candida tropicalis ATCC 7206 and C. albicans ATCC 10231, and 30% to C. albicans ATCC 98028. Organic extracts of the HAA-16 and HAA-17 strands exhibited noticeable anti-Candida activity, being the ethyl acetate extract of the HAA-17 strand the one with the highest performance thanks to a wider activity spectrum of MIC 3.25 mg/mL against C. albicans ATCC 7516 and Candida parapsilosis ATCC 7307. The selected actinomyces were identified by means of molecular techniques as members of the Streptomyces genus. Conclusions. Actinomyces associated to Atta cephalotes are excellent producers of bioactive compounds, being able to inhibit the growth of pathogenic mold of the Candida genus and with potential for application in the development of new natural products for the biomedical field.

Resumen La actinomicosis diseminada es muy infrecuente, así como la afección del sistema nervioso central (SNC) asociada, con mortalidad de hasta 28%. Sus manifestaciones pueden ser similares a cuadros infecciosos de otras etiologías, por lo que el conocimiento de la entidad aumenta la sospecha clínica y permite brindar un tratamiento oportuno. Se presenta el caso clínico de un adulto con edema en una extremidad superior como manifestación de una tromboflebitis y una lesión abscedada axilar, en que se confirmó una infección por actinomicetos. Presentó una diseminación hematógena con compromiso de SNC, de evolución fatal.


Actinomycosis is very rare, as well as the central nervous system (CNS) condition associated with it, presenting a mortality up to 28%. Its manifestations could be similar to infectious conditions from other etiologies, thus, having a better understanding of the entity increases clinical suspicion and also it can provide a timely treatment. The clinical case of an adult with edema in an upper extremity is presented as a manifestation of thrombophlebitis and an abscessed axillary lesion, in which actinomycetes infection was confirmed. He presented a haematogenous spread with CNS involvement, with fatal ending.

ABSTRACT

La amigdalitis hemorrágica es una complicación poco frecuente de los cuadros faringoamigdalinos con reducido número de casos reportados en la literatura. Presentamos el caso de una paciente de 28 años que consultó al servicio de urgencia por un cuadro de un día de evolución de odinofagia, fiebre y tos hemoptoica, diagnosticándose una amigdalitis hemorrágica que requirió amigdalectomía de urgencia por la persistencia del sangrado. En la biopsia se encontró la presencia de actinomices. Se manejó con terapia antibiótica por 14 días y evolucionó favorablemente sin complicaciones. Esa patología a pesar de ser un cuadro infrecuente, debe considerarse dentro de los posibles diagnósticos de sangrado de la vía aérea alta.


Hemorrhagic tonsillitis is a rare complication of tonsillitis, with only few case reports in the literature. We report the case of a 28-year-old patient evaluated at our emergency department because ofone-day sore throat, fever, cough and hemoptysis. Hemorrhagic tonsillitis was diagnosed that required emergency tonsillectomy that same day. The presence of Actinomyces was later described on the biopsy. Patient was managed with antibiotic therapy for 14 days and evolved favorably without complications. Despite being a rare sickness, hemorrhagic tonsillitis should be included in our differential diagnose of upper airway bleeding.

ABSTRACT

La actinomicosis es una enfermedad producida por bacterias del género Actinomyces. La forma torácica representa el 30% de los casos. La evolución es habitualmente crónica, con clínica variable. Se presenta una nina de 11 años de edad con tumefacción subescapular derecha de un mes de evolución, asociada a fiebre, hiporexia y pérdida de peso. Se diagnosticó actinomicosis torácica producida por Actinomyces meyeri. Recibió tratamiento antibiótico durante un año, con resolución completa. Aunque es poco frecuente, debe ser tenida en cuenta como diagnóstico diferencial en cuadros de tumoración torácica de larga evolución con compromiso del estado general. Debe informarse al microbiólogo la sospecha diagnóstica por lo difícil de su desarrollo en los medios de cultivo habitual.


Actinomycosis is a disease produced by bacteria of the genus Actinomyces. The thoracic form represents 30% of the cases. Signs and symptoms are generally unspecific. A previously healthy 11 year old girl was admitted with a chest mass of a month of evolution associated with fever, hyporexia and weight loss. Thoracic actinomycosis was diagnosed with the isolation of Actinomyces meyeri in the mass drainage. She received antibiotic treatment for a year with complete resolution. Although rare, it should be taken into account as a differential diagnosis in long evolution thoracic tumor affecting the general condition. The microbiologist must be warned about the suspected diagnosis because it is difficult to find it in routinely used culture media.

Introduction: Actinomycosis is an infrequent infection caused by bacteria from Actinomyces genus that manifests as a chronic, suppurative and progressive disease. Thoracic actinomycosis occurs in 18 percent of the cases, and infection by Actinomyces odontolyticus is even less frequent. The clinical presentation mimics tuberculosis or neoplastic processes. Clinical case: We report the case of a 75 years old man with COPD and Diabetes Mellitus type 2. He was referred to our clinic presenting a history of chronic cough, progressive dyspnea, fever and occasional bouts of haemoptysis. Chest radiograph showed a peripherally-located parenchymal opacity in the upper right lobe with over a year of evolution that later became a cavitary mass mimicking bronchogenic neoplasm or tuberculosis. The patient underwent bronchoscopic and CT- guided biopsy that showed necrosis and inflammatory cells. In the culture of cavitary fluids grew Actinomyces odontolyticus. We concluded that it was a thoracic actinomycosis. Penicillin 20 million units per day for six weeks was given, followed by oral amoxicillin for 6 months with good clinical and radiological response. Comments: To our knowledge this is the first report in Chile of lung infection caused by Actinomyces odontolyticus. Actinomycosis is a great masquerader, in this case we made the diagnosis with a fluid culture. This microorganism must be considered in the differential diagnostic in cavitary lung diseases.


Introducción: La actinomicosis pulmonar es una infección infrecuente causada por una bacteria del género Actinomyces, se manifiesta como un proceso crónico, supurativo de curso progresivo, el compromiso torácico ocurre aproximadamente en el 18 por ciento de los casos y la infección por Actinomyces odontolyticus es aun menos frecuente. Caso clínico: Se presenta el caso de un paciente hombre de 75 años de edad con antecedentes de EPOC y Diabetes Mellitus tipo 2, que fue derivado a nuestra clínica por cursar con una reagudización infecciosa persistente caracterizada por tos productiva, disnea progresiva,fiebre y episodios reiterados de hemoptisis de escasa cuantía. En la radiografía de tórax y tomografia computada, se detectaron opacidades mal definidas en lóbulo superior derecho de un año de evolución, que posteriormente se transforman en una masa cavitada adyacente a la pared toráxica simulando una neoplasia broncogénica o tuberculosis. El paciente fue sometido a fibrobroncoscopía realizándose biopsia y punción transbronquial. Posteriormente se efectuó biopsia por punción trans-toráxica guiada radiológicamente y en una muestra de tejido de aspecto necrótico y en líquido de la cavidad enviado a cultivo se pudo aislar Actinomyces odontolyticus. Concluyéndose que se trataba de una actinomicosis tóraco-pulmonar, se procedió a tratar con penicilina sódica 20 10(6) UI/dia por seis semanas y después se programó tratamiento por seis meses con amoxicilina vía oral, con buena respuesta clínica y radiológica. Comentarios: En nuestro conocimiento esta sería la primera comunicación en Chile de una lesión pulmonar producida por Actinomyces odontolyticus. La Actinomicosis, es un gran imitador, en este caso realizamos el diagnóstico con cultivo de líquido por punción. Este microorganismo debe ser considerado en el diagnóstico diferencial de lesiones cavitarias pulmonares.

ABSTRACT

Human actinomycosis with involvement of the spine is a rare condition although it has been first described a long time ago. It is probably underrecognized since its clinical presentation is often misleading and accurate bacteriological diagnosis is challenging. We herein report a rare case of cervical actinomycosis with paravertebral abscess and spondylitis imputed to an infection by Actinomyces meyeri in a 52-year-old immunocompetent Caucasian man. A. meyeri should be considered as a potential cause for subacute or chronic spondylitis, even in immunocompetent subjects. Modern diagnostic tools such as Matrix-Assisted Laser Desorption–Ionization Time of Flight mass spectrometry and 16S rRNA sequencing are efficient for accurate microbiological identification.

This study investigated the presence of the black-pigmented bacteria Prevotella nigrescens and Prevotella intermedia, the non-black-pigmented bacteria Actinomyces spp and particularly the cariogenic pathogen Streptococcus mutans in the dental biofilms of patients with or without black extrinsic tooth stains, using the multiplex polymerase chain reaction (PCR) technique. Analysis of the dental biofilms of patients with (n=26) or without (n=26) black tooth stains was performed using duplex PCR for the 16S ribosomal RNA gene (P. nigrescens, P. intermedia, Actinomyces spp) and glucosyltransferase-I gene for S. mutans. P. nigrescens and S. mutans were the most frequent bacteria detected in both groups. The least frequently detected were P. intermedia and Actinomyces spp. The similar bacterial composition of dental biofilms of black tooth stains and healthy tooth surfaces indicates that black tooth stains are not free of cariogenic bacteria.


O objetivo deste estudo foi investigar a presença das bactérias pigmentadoras de negro Prevotella nigrescens e Prevotella intermedia, da não pigmentadora de negro Actinomyces spp e particularmente a bactéria cariogênica Streptococcus mutans, no biofilme dentário de pacientes com ou sem manchas dentárias extrínsecas negras, utilizando a técnica multiplex PCR (reação em cadeia da polimerase). Análises do biofilme dentário de pacientes com manchas (n=26) e sem manchas (n=26) foram realizadas utilizando a multiplex PCR para o gene 16S RNA ribosomal (P. nigrescens, P. intermedia, Actinomyces spp) e o gene glucosiltransferase-I para S. mutans. P. nigrescens e S. mutans foram as bactérias mais frequentemente detectadas em ambos os grupos. As menos frequentemente detectadas foram P. intermedia e Actinomyces spp. A similaridade entre a composição bacteriana dos biofilmes dentários das manchas dentárias extrínsecas negras e das superfícies dentárias sem manchas indicam que as manchas dentárias extrínsecas negras não estão livres de bactérias cariogênicas.

ABSTRACT

A canaliculite é uma infecção rara e crônica do canalículo lacrimal, cuja etiologia mais comum é o Actinomyces israelli (bactéria gram positiva, anaeróbica). Esta afecção não apresenta cura espontânea. O diagnóstico diferencial se faz com: conjuntivite crônica recorrente, blefarite e hordéolo. O tratamento definitivo é o cirúrgico (canaliculotomia), sendo realizado sob anestesia local.


The canaliculitis is a rare infection and chronicle lachrymal pathology, whose commoner etiology is Actinomyces israelli ( bacteria anaeroby positive gram). This pathology does not present cure espontany The diagnosis differential with is done: Recurring chronic conjunctivitis, blefarity and hordeoly. The definitive treatment is the surgical (canaliculotomy)), being accomplished under location anesthetizes.

ABSTRACT

Actinomyces strain A01 was isolated from soil of a vegetable field in the suburb of Beijing, China. According to the morphological, cultural, physiological and biochemical characteristics, and 16S rDNA sequence analysis, strain A01 was identified as Streptomyces lydicus. In the antimicrobial spectrum test strain A01 presented a stable and strong inhibitory activity against several plant pathogenic fungi such as Fusarium oxysporum, Botrytis cinerea, Monilinia laxa, etc. However, no antibacterial activity was found. In pot experiments in greenhouse, the development of tomato gray mold was markedly suppressed by treatment with the fermentation broth of the strain A01, and the control efficacy was higher than those of Pyrimethanil and Polyoxin. A main antifungal compound (purity 99.503 percent) was obtained from the fermentation broth of strain A01 using column chromatography and HPLC. The chemical structural analysis with UV, IR, MS, and NMR confirmed that the compound produced by the strain A01 is natamycin, a polyene antibiotic produced by S. chattanovgensis, S. natalensis, and S. gilvosporeus, widely used as a natural biological preservative for food according to previous reports. The present study revealed a new producing strain of natamycin and its potential application as a biological control agent for fungal plant diseases.


A cepa Actinomyces A01 foi isolada do solo de um campo agrícola no subúrbio de Beijing, China. De acordo com as características morfológicas, culturais, fisiológicas e bioquímicas, e análise da sequência 16S rDNA , a cepa A01 foi identificada como Streptomyces lydicus. Nos testes de espectro antimicrobiano, a cepa A01 apresentou atividade inibitória intensa e estável contra vários fungos patogênicos para plantas, como Fusarium oxysporum, Botrytis cinerea, Monilia laxa, etc. Entretanto, não foi encontrada atividade antibacteriana. Em experimentos em estufas, o desenvolvimento do fungo cinza do tomate foi fortemente inibido pelo tratamento com o caldo de fermentação da cepa A01, com eficiência superior à do pyremethanil e polyoxin. Por cromatografia em coluna e HPLC, obteve-se um composto fúngico (pureza 99,503 por cento), cuja análise estrutural por UV, IR, MS e NMR revelou ser natamicina, um antibiótico polienico produzido por S. chattanovgensis, S. natalensis e S.gilvosporeus, empregado como conservador biológico natural em alimentos. O presente estudo relata a detecção de uma nova cepa produtora de natamicina e sua aplicação potencial como um agente de controle biológico de doenças fúngicas em plantas.

ABSTRACT

Primary actinomycotic endocarditis is extremely rare. The author reported a case who suffered from endocarditis due to Actinomyces georgiae which the portal of entry for the organism could not be determined The author also reviewed the literature which described similar conditions. In addition, to the author's knowledge, there have been no reported cases of actinomyces georgiae endocarditis. Thus, this is the first report in the world. Clinical features of this rare condition are indistinguishable from other bacterial endocarditis. Actinomyces spp. is usually susceptible to a wide range of antibiotics, and endocarditis caused by this genus needs a prolonged course of antimicrobial treatment. However the optimal duration of therapy is still unknown. Prognosis is good with early detection and proper management.

ABSTRACT

Actinomycosis caused by Actinomyces spp. is a chronic and suppurative infection caused by an endogenous gram positive bacterium. The unusual sites of infection are the head and neck, thorax and abdomen and are almost always endogenous in origin. Primary cutaneous actinomycosis is very rare and is usually associated with external trauma and local ischemia. We report a case of a primary cutaneous actinomycosis of the thigh in a 30-year-old man. The patient acquired the infection through an injection wound which progressed to multiple discharging sinuses. Clinical material from the wound demonstrated the presence of Actinomyces in smears and cultures. The patient was diagnosed and successfully treated with surgical resection and combined antibiotic therapy.

ABSTRACT

Histopathologic analysis of 73 cases of mycetoma occurring in western Rajasthan was done from January 2001 to December 2005. Maduromycotic mycetoma remains commonest in this region as compared to actinomycotic mycetoma, which is more common in the southern part of Rajasthan. The incidence of actinomycotic mycetoma has increased during the last five years in this part of Rajasthan due to changes in climatic conditions, like heavy rainfall, increased irrigation by Rajasthan Canal, urbanization of villages, and modification in agriculture. The ratio of prevalence of maduromycotic mycetoma to the prevalence of actinomycotic mycetoma has decreased from 4:1 to 1.91:1 during the last five years in western Rajasthan.

ABSTRACT

La infección actinomicótica del corazón es una enfermedad poco común, especialmente cuando el compromiso endocárdico valvular es primario. Sólo unos pocos casos han sido publicados. Comunicamos un caso de endocarditis primaria por Actinomyces sp de la válvula mitral, diagnosticada en la necropsia de un hombre de 34 años con antecedentes de valvulopatía reumática crónica, que falleció con una endocarditis infecciosa.


Actinomycotic infections of the heart is an uncommon disease, especially if the infection affects the valvular endocardium as primary focus. Just a few cases have been reported previously. We report a case of primary endocarditis of the mitral valve caused by Actinomyces sp diagnosed at necropsy in a 34 year-old man with history of chronic rheumatic disease presenting as a usual case of infective endocarditis.
